IMPORTANT NOTICE - Retirees Must Read

In order to file for Medicare and to be seen expeditiously please make an appointment by calling 1-800-772-1213. You can make an appointment with your local office or the United Nation Grand Central office (if you live in NY). On your assigned appointment you will need to bring the following:

• Birth Certificate
• 2 separate pieces of IDs issued at least 5 years ago (ex. Passport and Driver License)
• Certificate of Naturalization or USA passport
• I551 card (Green Card's official name) with proof of 5 years of continuance residency from present.


Medicare Part B

Effective 01 January 2011 the UNHQ Medical Programme will now require all eligible ASHI participants who have not yet attained age 75 as of 01 January 2011 to enroll in the US Medicare B Program in an effort to contain medical cost.

Medicare Part D Notice

The prescription drug benefits provided by the UNHQ Medical Programme is considered to be
"Creditable Prescription Drug Coverage". Therefore ASHI participants do not need to enroll in Medicare Part D.
